Long summary:Instructions for Completing ATA Continuing Education Approval Request Form Page 1Instructions for Completing ATA Continuing Education Approval Request Form General Information: ATA maintains a database of approved events at which ATAcertified members may earn continuing education points (CEPs). For events not listed an ATA approval request must be completed and submitted to ATA Headquarters. Approval may be requested either prior to an event or after an event with the understanding that the approval may be denied if documentation is insufficient or if the educational content does not meet ATA criteria. Individuals and groups requesting CEPs will be notified by ATA Headquarters that the event has been approved for a particular number of CEPs or that approval is denied. Individuals must keep track of their earned CEPs and report them to ATA Headquarters every three years upon request. Select one of the following forms to complete: 1. If you represent a chapter regional group organization institution or other sponsor of activities complete the ApprovalRequest Form for Groups. 2. If you are an individual complete the Approval Request Form for Individuals. CEP Request Form for Groups1) Provide the name and contact information for the group sponsoring the event. a) Check the appropriate box for your group and provide the groups name. b) Other can include affiliated groups international translation organizations and universities. All ATA Chapter educational events are automatically eligible for continuing education points. Events not sponsored by ATA or ATA Chapters must be approved individually.
